Reactions 724 - 24 Oct 1998 S Octreotide First report of cluster headache: case report A 46-year-old woman developed cluster headaches after she started SC octreoide 100µg every 12 hours to decrease her levels of growth hormone. She had undergone a pituitary adenomectomy for acromegaly 16 years earlier and had persistently high levels of growth hormone. After the woman began octreotide therapy, she gradually developed pulsatile headaches in her right frontal and parietal area, in addition to right-sided rhinorrhoea and lacrimation. Her headaches occurred 10–15 times daily and lasted for about 30–60 minutes. However, a few minutes after each octreotide injection, her headache disappeared, often for 6–8 hours. Relief from headaches required increasingly frequent octreotide injections and in severe periods the headache attacks occurred every day for 3–4 months. Two years after she started the octreotide injections, the woman’s level of growth hormone normalised. [Patient outcome not stated.] Author comment: Octreotide appeared to exert both algogenic and analgesic effects in this patient. Otsuka F, et al. Cluster headache dependent upon octreotide injection.
