Summary: what is the best approach to AML in older patients?
• Unmet medical need for treatment options in older AML patients (increasing number of patients; no improvement in survival last decades)
• Treatment should be considered in (almost) all patients: intensive chemotherapy / LD-Ara-C / GO / hypomethylating agents (all shown to be superior to BSC)
• Intensive treatment is the preferred treatment; except for: o Unfit patients (e.g. HCT-CI≥3; SPPB <9; 3MS < 77)o Poor risk disease (e.g. poor risk cytogenetics)
• For patients in CR(i) allogeneic hematopoietic cell transplantation should be considered
• Future studies are needed, to compare:o Intensive treatment with hypomethylating agents (ongoing EORTC/GIMEMA)o Hypomethylating agents with LD-Ara-C
• Future studies are needed to combine new agents with low toxicity regimen (e.g.HOVON135)
