Reactions 1384 - 14 Jan 2012 S Octreotide First report of mesenteric ischaemia: case report A 62-year-old woman, whose history included coronary artery disease, underwent coronary artery bypass grafting and started receiving SC octreotide 100µg three times daily for chylomediastinum on postoperative day 7. She developed loose, watery stools and mild rectal bleeding 2 days later. The following day, right lower quadrant pain with guarding and rebound tenderness emerged, and her WBC count increased from 16 to 40.2/mm 3 . A diagnostic laparoscopy showed caecal ischaemia, and she underwent caecectomy. She recovered, and her chylomediastinum spontaneously resolved over the following week. Author comment: "It is possible that the patient described above suffered from mesenteric vascular disease in addition to coronary artery disease. The reduction of splanchnic blood flow by octreotide may have precipitated mesenteric ischemia in this patient." Rotman S, et al.
