Three concluding remarks made to the Commission (Statement to National Commission on Deepwater Horizon Oil Spill and Offshore Drilling, Nov.9, 2010)

1. A culture of safety has to be born within the organization. You cannot buy culture. You have to make it yourself.

2. Creating a strong, sustainable safety culture is a long process. If an organization is truly going to overhaul its approach to safety, it has to be committed from day one. But, you can’t start until you start — and you’re never going to finish.

3. The first and last elements — the bookends of OIMS — are the most critical. These are “Management Leadership and Accountability”, and “Operations Integrity Assessment and Improvement”. Without leadership by example and without thoughtful, honest and objective self-assessment, no system is sustainable.

Liquid Overfill Risk Analysis ToolBackground

• Following the BP Texas City and Buncefield explosions in 2005, ExxonMobil began a systematic evaluation of the potential for similar events in its manufacturing operations worldwide

• A phased approach was utilized, focusing first on the potential for vapor and two-phase releases through atmospheric relief systems

• A final phase is currently focused on the potential for liquid overfill releases from process vessels and atmospheric storage tanks

• Consistent with our risk management practices, a risk assessment and risk-based decision making process is being used

• A risk assessment tool was developed specifically for liquid overfill releases

• The aim of this presentation is to provide an overview of the Liquid Overfill Risk Assessment Tool (LORAT) and the methodology used for assessing vessel and tank overfill risks

• Incorporates probability and consequence parameters

− Probability factors were developed and validated for operator intervention, ignition probability and people in area

− Modeling equations used to estimate flash fraction, aerosol generation, liquid rainout, falling liquid/splash zone evaporation, liquid pool evaporation and vapor dispersion

• Event tree determines frequency of outcomes for each scenario, which can be aggregated per vessel or tank

Flexicoker LPG transportVan weg naar water• Veiligheid heeft bij ExxonMobil de hoogste prioriteit.

• Dit project heeft eenvijfde van alle LPG transport over de weg in Nederland weggenomen.

• Het Flexicoker LPG transport over de weg (van Rotterdam naar Antwerpen) gedurende ruim 12 jaar was in termen van veiligheid beter dan alle LPG transport in Nederland.

• ExxonMobil schakelt over omdat vervoer per boot een aanzienlijk lager risico heeft.

• “Veiligheid Voorop” is een gezamenlijk initiatief, dat er toe moet leidendat een solide veiligheidscultuur de standaard wordt in alle bedrijvendie grootschalig met gevaarlijke stoffen werken.

• Wat zijn de kernelementen ?1. Betrokken leiderschap2. Excellente VBS uitvoering

a. continue verbeteringb. meten veiligheidsprestatiesc. versterken competenties

3. Actieve veiligheidsnetwerken4. Leden doen zaken met bedrijven, die ook een solide veiligheidscultuur hebben

• Hoe liggen de verantwoordelijkheden ?� Individueel bedrijf blijft verantwoordelijk voor eigen veiligheidsprestaties� Branchevereniging bevordert en ondersteunt met uitvoeing van 10 actiepunten.

Heeft coordinerende rol voor rapportage m.b.t. 10 actiepunten

• Tevens leent dit “initiatief” zich om de samenleving de inspanningen en ontwikkelingen op veiligheidsgebied te laten zien.

• Pro-actieve deelname aan nieuwe ontwikkelingen :� ontwikkeling van nieuwe wetgeving en richtlijnen (bijv. PGS-29)� implementatie van internationale standaards (bijv. API) � bevordering van goede brancherichtlijnen (bijv. SIR) � versterken competenties (bijv. VCA systeem)

• Every company desires safe operations — but the challenge is to translate this desire into action.

• The answer is not found only in written rules, standards and procedures. While these are important and necessary, they alone are not enough.

• The answer is ultimately found in a company's culture — the unwritten standards and norms that shape mindsets, attitudes and behaviors.

• Companies must develop a culture in which the value of safety is embedded in every level of the workforce, reinforced at every turn and upheld above all other considerations.

Samenvatting /Stellingen

• De integriteit van een proces wordt niet bepaald door een BRZO audit.

• Nul ongevallen kun je niet bereiken met alleen veiligheidsregels en procedures.

• Veiligheidscultuur is niet te koop bij consultants, maar moet een bedrijf zelf organiseren.

• Steeds vaker wordt er op middelvoorschriften geaudit.

• Middelvoorschriften zijn makkelijk te controleren voor een auditor, maar dragen niet noodzakelijkerwijs bij aan betere veiligheid.

• Bedrijf is verantwoordelijk voor veiligheid, overheid toetst op risico gebaseerde aanpak.
