Historische trends in zorguitgaven Ontwikkeling in publieke zorguitgaven
(aandeel in GDP, 1960 – 2011)Publieke uitgaven aan zorg:
•2011 7.8% van GDP 1980 5.7% van GDP.
•2011 15% publieke uitg. 1980 11.5%
-> Crowding out van andere belangrijke uitgaven-categorieën. •Sterke stijging vanaf 2000 met vertraging/krimp tijdens de crisis.
•Sinds 2010 nemen veel lidstaten maatregelen om financiering te veranderen om besparingen te realiseren.

De stijging van het inkomen per hoofd verklaart ongeveer 60% van de totale uitgavenstijging,
Prijseffecten dempten de groei met zo'n 18%,
Veranderingen in demografie nemen slechts 7% van de stijging voor hun rekening,
Overige effecten, (bijv. technologische ontwikkeling, beleidsmaatregelen) staan garant voor circa 50%.
Trends uit het verleden worden dus vooral verklaard door niet- demografische factoren, inclusief inkomens- en prijseffecten.

Uitgaven aan zorg: waarom een fiscaal risico?
60-70% van de toename tussen 2010 en 2060 in leeftijdgerelateerde uitgaven is toe te schrijven aan gezondheidszorg en langdurige zorg.
Kostenstijging vooral door non-demografische oorzaken: nationaal inkomen, medische technologie, institutionele vormgeving van zorgstelsels, en voor een beperkt deel dus ook veroudering van de bevolking.
Daarom is het belangrijk om bij houdbaarheidsdiscussie de gezondheidszorg te betrekken.
Hoeveel wordt uitgegeven aan zorg is een politiek/maaschappelijke keuze, met fiscale houdbaarheid als belangrijk ankerpunt.

De EU en gezondheidszorg Beperkte rol:
Het staat duidelijk in het verdrag:Health policy and the organisation, financing and management of healthcare is a national responsibility of member countries (Article 168 of the Treaty on the functioning of the European Union).
Maar, de EU heeft een rol:
Bevordering van samenwerking en coordinatie tussen lidstaten.
Grensoverschrijdende zorg, interne markt, onderwijs, eHealth, onderzoek.
Versterkte controle van de uitgaven voor gezondheidszorg is een van de elementen van het vernieuwde begrotingstoezicht ter voorkoming van toekomstige economische/fiscale crisis. 12

Europese Semester – gezondheidszorg en langdurige zorg
wordt steeds belangrijker.
In 2012, 6 EU lidstaten ontvingen aanbevelingen:
Oostenrijk, België, Bulgarije, Cyprus, Duitsland en Nederland.
In 2013, 15 EU lidstaten:
Oostenrijk, België, Bulgarije, Tsjechië, Finland, Frankrijk, Duitsland, Luxemburg, Malta, Nederland, Polen, Roemenië, Slovakije, Slovenië en Spanje.
