Aanleiding - casusMan 56, alcohol + nicotine abusus A/ 1 week bovenste LWI, 5 dagen keelpijn + slikklachten, 1 dag zwelling keel LO/ roodheid, zwelling & drukpijn hals. MK holte cysteuze afwijking tonsilnis, asymmetrie hypopharynx Flexibele laryngoscopie: opvulling gehele sinus piriformis links, ruimtewerking vanuit para- en retrofaryngeaal CT/ Beeld van peritonsillaire en retrofaryngeale abcessen beiderzijds. Induratie carotisloge rechts, geen thrombus. Induratie vetweefsel bovenste mediastinum zonder vochtcollecties.
C/ abces retro+parafaryngeaal B/ Ceftriaxon+metronidazol, drainage op OK, postop. geïntubeerd naar IC !OK/ (KNO) Tonsillectomie, drainage parafaryngeaal abces via tonsilloge, drainage retrofaryngeale ruimte en paralaryngeaal !Beloop/ Kweken: streptococcus constellatus (BK + abces) Uitbreiding naar mediastinaal, in totaal 3x re-operatie Succesvolle detubatie 16 dagen na initiële operatie Ontslag naar low care met AB

Oppervlakkig = m. trapezius + sternocleidomastoideus, glandula submaxillaris + parotis Midden = pharynx, oesophagus, larynx, trachea, thyroid, bijschildklieren Diep = wervelkolom + spieren

InfectiesRetropharyngeaal Lokaal (achterwand oesofagus bij corpus alienum/complicaties endoscopie) Uitbreiding van parafaryngeaal !Danger space Uitbreiding retropharyngeaal !Prevertebraal infectie wervelkolom (discitis, osteomyelitis)

VerwekkersPolymicrobiëel (>4) Anaëroben : aëroben = 10:1 !Anaeroob - Grampos peptostreptococ - Gramneg actinomyces, fusobacterium, bacteroides, prevotella !Aeroob - Grampos streptococ, stafylococ*, lactobacillus, corynebact. - Gramneg moraxella, enterobacter, pseudomonas*
* = vaker bij pt met risicofactoren (immuungecompromitteerd, DM, postop)
Behandeling- Airway management - Antibiotica (UMCN: vlgs Swab richtlijn = augmentin of penicilline + metronidazol, 2 weken) (Up to date: 2-3 weken en ‘langer’ bij complicaties) !
- Initiële fase van cellulitis = geen chirurgie - Drainage alleen bij abcesvorming, necrotiserende mediastinitis • Cervico-mediastinale benadering – vroege mediastinitis • Uitbreiding in danger space – thoracotomie
Complicaties- Betrokkenheid van carotis loge
- Mycotisch aneurysma a. carotis – ruptuur - Uitbreiding craniaal
- Suppuratieve thromboflebitis v. jugularis (syndroom van Lemierre) - Necrotiserende mediastinitis (soms retroperitoneale uitbreiding) - Uitbreiding naar pleura + empyeem - Uitbreiding naar pericard + tamponade
Syndroom van LemierreAka Necrobacillose , post-angina sepsis In 1900 voor het eerst beschreven !Kenmerken van klassieke vorm 1) Oropharyngeale infectie 2) Positieve bloedkweken Fusobacterium necrophorum 3) Klinische verdenking/bewezen thromboflebitis v. jug. Interna 4) Septische embolieën elders (long)

KliniekTypisch bij jonge patiënten (15-20 jaar) Onset van koorts, KR en sepsis 4-5 (12) dagen na episode van keelpijn - Pijnlijke e/o stijve nek, zwelling/drukpijn in de hals (thrombus v. jug) - Lymfadenopathie - Klachten t.g.v. metastatische abcessen
- Thoracale pijn, hemoptoë (empyeem 10-15%) - Gewrichtsklachten – septische arthritis - Buikpijn – lever/milt abcessen - Meningitis, cerebrale abcessen - Endocarditis zeldzaam !
Fusobacterium - Normale flora mond-keelholte - Infectie voorafgegaan door virale – EBV infecties? (adolescenten?) - Subspecie fusobacterium necrophorum heeft eigenschappen thrombocytenaggregatie + LPS endotoxine !
Afname van incidentie sinds gebruik van antibiotica (jaren ’60 - ’70) !Sinds jaren ’90 weer toename - Terughoudender gebruik van antibiotica bij BLWI? - Fusobacterium vaker (o.a erythromycine) resistent?
!Nog steeds zeldzaam (incidentie 1:1.000.000) !4-6 weken AB (metronidazol/clinda gevoeligheid Fusobacterium +) Gebruik van antistolling blijft onderwerp van discussie (expert opinion: niet, tenzij hoog risico/uitgebreid thrombus) !Mortaliteit 5% (tot 18%)
