De methode waarbij Lasers of Intense Pulsed Light (IPL) selectief specifieke targets in de huid vernietigen, waarbij het risico op

schade van omliggende structuren wordt geminimaliseerd.

– Lasers die werken met golflengtes van licht in de buurt v.d. primaire absorptie pieken van Oxyhemoglobine

• Risico’s: depigmentatie, blaarvorming, ulceratie, verlittekening

• Oorzaak onbekend

• Geen ziekte van de talgklieren • Geen verhoogde talgproductie

• Al lijken talgklieren wel een essentiële rol te spelen:

– Voorkeuslocaties hebben verhoogd aantal + grotere talgklieren

• Rol Malassezia (Pityrosporum ovale, gist) onduidelijk

– Scherp afgrensbare erythemateuze plaque met een vettige/gelige schilfering

– Voorkeurslocaties: • Scalp • Centraal in het gezicht • Externe gehoorgang • Bovenste gedeelte romp • Huidplooien

• Scalp/retro-auriculair:

– Mild: Pityriasis sicca • Fijne witte schilfering, diffuus, zonder erytheem • Asymptomatisch / pruritus •

– Ernstiger: Pityriasis steatoides • Onregelmatige oranje- tot zalmkleurige plaques met

gelige/vettige schilfering • M.n. tempoparietaal

Eczema Seborrhoïcum Romp: 5 Verschillende distributiepatronen.

Locatie:

1. Nattend erythemateuze intertrigo

Oksels, borstplooien, navel, liezen

2. Petaloïd patroon: polycyclische fijn schilferende dunne plaques

Sternaal, interscapulair

3. Annulaire/boogvormige ronde tot ovale licht schilferende plaques, soms met centrale hypopigmentatie

4. Pityriasiform patroon: 5-15mm ovale schilferende laesies Gedistribueerd langs de huidspanningslijnen

5. Psoriasiform patroon: grote ronde erythemateuze plaques met dikke schilfering

Bij HIV/aids kan het zeer uitgebreid zijn en op ongewone locaties zoals de extremiteiten zitten

• Evt. combinatiepreparaat met Salicylzuur 5% of 10%

– Topicorte lotion 1dd • Evt. combinatiepreparaat met Salicylzuur 5% of 10%

– Antimycotica: • Ketoconazol cr, Loprox cr, Myk cr, Lamisil cr

– Corticosteroïd klasse I of II • Hydrocortison 1% cr, Triamcinolon 0,1% cr • Terughoudend met sterker corticosteroïd i.v.m. rebound effect

– Calcineurineremmers • Elidel crème, Protopic zalf
