Gelukkige gezichten na een leerzame cursus

Het zal me benieuwen, zoals Sonneveld zei. Van 1988
tot nu werd het ministerie van volksgezondheid en ook
sociale zaken beheerd door een socialist en bij
uitzondering in de paarse periode door een groene.
Na 25 jaar kunnen we wel vaststellen dat er weinig
beweging zat in dit departement. Naast veel
regelgeving werd weinig fundamenteel veranderd, laat
staan verbeterd. Zeker de laatste jaren zijn de regeltjes
en verplichtingen die eigenlijk niets met tandheelkunde
te maken hebben, de grootste verandering geweest. De
titel “algemeen tandarts”, wachtdiensten zonder
evaluatie, accreditering zonder evaluatie… Nu, met de
invoering (verplicht) van een derde betalingssysteem,
een kwalijk ruikend systeem met excessief frauderen, is
er een volgende stap gezet in het extra beladen met
administratie. Wie protesteert? Ik hoor en zie weinig
beweging. Informatie is schaars tot onbestaande.
Tegemoetkoming in de extra rompslomp onvindbaar.
Nu er een liberaal, geneesvrouw en dus deskundige,
aan het roer komt, zijn de verwachtingen hoog denk ik.
Wie plaatst de tandheelkunde eindelijk in een passend
daglicht als onderdeel van volksgezondheid? Wie eist,
zoals de huisartsen vroeger deden, een verantwoord
ereloon? Wie evalueert eindelijk de 15 jaar
accreditering? Het zal me benieuwen….

Abstract OBJECTIVES: Research suggests that varied etiologic factors are responsible for burning mouth syndrome (BMS). We examined the role of immune and endocrine function in the pathology of BMS. METHODS: We conducted a case-control study to evaluate immune (lymphocyte subpopulations) and endocrine (hypothalamus-pituitary-adrenal axis and sympathetic-adrenomedullary system) function in 47 female BMS patients and 47 age-matched female controls presenting at an university clinic. Psychological state was assessed with the Zung Self-Rating Depression Scale and Taylor Manifest Anxiety Scale. RESULTS: BMS patients were significantly more anxious than controls (P=0.011). Plasma adrenaline level was significantly lower (P=0.020) in BMS patients than in controls, and linear regression analysis of all patients combined revealed that depression level was significantly positively associated with plasma noradrenaline and cortisol levels (P=0.002 and 0.001, respectively). However, as compared with controls, BMS patients had a significantly lower CD8(+) cell count (P<0.001) and a significantly higher CD4/CD8 ratio (P=0.002). Discriminant analysis revealed that CD8(+) cell count and CD4/CD8 ratio were independent variables that distinguished BMS patients from controls. DISCUSSION: The immunoendocrine system is substantially involved, and may have a key role, in the mechanism of chronic pain in BMS patients. Immune function was significantly and specifically suppressed in BMS, although the hypothalamic-pituitary-adrenal axis and sympathetic nervous system were predominantly activated by psychological stress that was not specific to BMS.

Abstract AIM: Allergy to materials used during root canal treatment is well recognized in the endodontic literature. However, allergy to Ledermix paste, a compound containing triamcinolone (a corticosteroid) and demeclocycline (a tetracycline antibiotic), has not been reported apart from one very recent case in a Letter to the Editor of a journal. The aim of this report is to describe a proven allergic reaction to tetracycline following the use of Ledermix paste as a root canal medicament. SUMMARY: A 33-year-old female patient undergoing root canal retreatment of her right mandibular second premolar tooth experienced symptoms of type 1 allergy following the placement of an intracanal medicament containing a mixture of Ledermix paste and calcium hydroxide [Ca(OH)2 ]. Signs of the type 1 allergy included urticaria, pruritus all over the body, general malaise and fever. These all subsided after removing and flushing out the Ledermix paste-Ca(OH)2 mixture and redressing the root canal with Ca(OH)2 alone. Allergic scratch tests were performed, and these confirmed that the patient was allergic to tetracycline, one of the components of Ledermix paste. © 2014 International Endodontic Journal. Abstract Background: Long-term studies worldwide indicate that peri-implant inflammation is a frequent finding and that the prevalence of peri-implantitis correlates with loading time. Implant loss, while less frequent, has serious oral health and economic consequences. An understanding of predictive factors for peri-implant disease and implant loss would help providers and patients make informed decisions. Methods: A cross-sectional study was performed on 96 patients with 225 implants who had their implants placed between 1998 and 2003. Implant placement data was collected from patient records and patients presented for a clinical and radiographic follow-up examination. Implant status and periodontal status were determined and the data analyzed to determine prevalence of peri-implant disease or implant loss and a predictive model was tested. Results: The mean follow-up time for the patients was 10.9 years. The implant survival rate was 91.6%. Peri-implant mucositis was found in 33% of the implants and 48% of the subjects and peri-implantitis occurred in 16% of the implants and 26% of the subjects. Subjects with peri-implantitis were twice as likely to report a problem with an implant as subjects with healthy implants. Peri-implantitis is associated with younger ages and diabetes at time of placement, and with periodontal status at time of follow-up. Implant loss is associated with diabetes, immediate placement and larger diameter implants. Conclusions: One in four patients and one in six implants have peri-implantitis after 11 years. The data suggests that periodontal and diabetes status of the patient may be useful for prediction of implant outcomes.
Clin Oral Implants Res. 2014 Nov 19. What is the impact of bisphosphonate therapy upon dental implant survival? A systematic review and meta-analysis. Ata-Ali J1, Ata-Ali F, Peñarrocha-Oltra D, Galindo-Moreno P.
Abstract OBJECTIVE: A systematic review and meta-analysis are carried out to assess the scientific evidence that bisphosphonate therapy can decrease the success rate of dental implants. MATERIAL AND METHODS: The PubMed (Medline) database was used to search for articles published up until February 22, 2014. The meta-analysis was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA). The Newcastle-Ottawa scale (NOS) was used to assess study quality. RESULTS: The combinations of search terms resulted in a list of 256 titles. Fourteen finally met the inclusion criteria and were thus selected for inclusion in the systematic review. Eight studies (six retrospective and two prospective) were included in the meta-analysis, with a total of 1288 patients (386 cases and 902 controls) and 4562 dental implants (1090 dental implants in cases and 3472 in controls). The summary odds ratio (OR = 1.43, P = 0.156) indicates that there is not enough evidence that bisphosphonates have a negative impact upon implant survival. According to the number need to harm (NNH), over 500 dental implants are required in patients receiving bisphosphonate treatment to produce a single implant failure. CONCLUSION: Our results show that dental implant placement in patients receiving bisphosphonates does not reduce the dental implant success rate. On the other hand, such patients are not without complications, and risk evaluation therefore must be established on an individualized basis, as one of the most serious though infrequent complications of bisphosphonate therapy is bisphosphonate-related osteonecrosis of the jaws. Given the few studies included in our meta-analysis, further prospective studies involving larger sample sizes and longer durations of follow-up are required to confirm the results obtained.

Een mooi gebit een waardevol bezit?In de huidige samenleving speelt de esthetiek een steeds be-langrijkere rol hetgeen ook duidelijkeconsequenties heeft voor de Tandheelkunde. Dacht men vroeger bij functie van het gebit vooral aan de kauwfunctie tegenwoordig spelen esthe-tische aspecten eveneens een belangrijke rol. De hedendaagse patiënt is mondig en krijgt via interneten televisie al snel het idee dat alles maakbaar is. In deze samenspraak komen allerlei aspecten van de tandheelkundige esthetiek aan bod en wordt belicht hoe daar als tandarts deontologisch correct mee om te gaan.
Wat is mooi? Plastische chirurgie en verfraaiingen van het uiter-lijk zijn tegenwoordig helemaal ‘in’. Ook in de tandartspraktijk worden we dagelijks geconfronteerd met de vraag hoe we de patiënt aantrekkelijker kunnen maken. Maar wat is mooi precies?
Hoe om te gaan met een esthetische hulpvraag?Hoe te reageren op de vraag of jij als behandelaar behan-deling ook noodzakelijk acht? Wanneer wordt de eigen li-chaamsbeleving ongezond? En hoe stel je dat vast? Moet je als tandarts altijd ingaan op de wens van de patiënt, en wan-neer niet? Moet je wachten tot de patiënt zelf met een vraag komt of mag/moet je de patiënt daarover informeren?
Wat is het verschil tussen compromisloze enpragmatische esthetiek en in hoeverre speelt hetdynamisch behandelconcept hierin een rol? Hoe tereageren op de vraag of jij als behandelaar debehandeling ook noodzakelijk acht? Hoe moet je depatiënt voorbereiden op het resultaat direct na debehandeling en om teleurstelling te voorko-men? Wat te doen als de patiënt het resultaat van de behandeling te mooi vindt?
