• Niet alle trekkingen gaan gepaard met epileptische activiteit op het EEG
• Niet alle epileptische activiteit op het EEG gaat gepaard met waarneembare klinische trekkingen
Eerste patiënt die ik ooit zag op de IC van de VU…
Man van ongeveer 65 jaar Status na circulatiestilstand en reanimatie
In coma, en enorme myoclonieën na stimulatie
Casus
• 62 jarige man • Circulatiestilstand, werd waargenomen • Onmiddellijk BLS • Bij aankomst ambulance VF • Meteen gedefibrilleerd • tijd tot ROSC <10 minuten
Casus
• GCS bij opname gescoord als 3 • Echter, in de gegevens terugkijkend kan
het E2M4V1 geweest zijn, met onrust en gasping.
• Patiënt kreeg sedatie in ambulance

• Patiënt werd opgenomen op de IC • Gekoeld tot 36°C
Casus
• Op dag 2 zag ik de patiënt, en noteerde 'intermitterend myoclonus'.
• Myoclonus was wel zeer frequent aanwezig, zowel spontaan als wel uitgelokt door stimuli
• Patiënt kreeg Midazolam en Propofol. .

Patiënt maakte ook kauwende en smakkende bewegingen.

• Op dag 3 was het klinische beeld hetzelfde, ondanks ophogen van Midazolam en Propofol.
• Neuroloog zag patiënt op dag 3 en dacht ook aan myoclonus, maar ook aan rillen en epilepsie.

• Het EEG toonde zowel snelle als trage ritmes, soms onregelmatig, maar geen evidente epilepsie.
• Tijdens het maken van het EEG waren er spiertrekkingen, geduid als myoclonieën.

• Patient werd wakker op dag 5 • Hij was goed georiënteerd en werd
ontslagen naar de CCU • Hij vertoonde geen myoclonieën meer • Hij kreeg een ICD
Casus
• De revalidatiearts die hem later zag, noteerde enige EPS: traag, enige rigiditeit.
• Dit werd geweten aan de haldol waarmee zijn later ontstane delirium behandeld was
• Na stoppen van de haldol verdwenen de EPS

• Zijn cognitie was goed. Ook geheugen, reality-testing en executieve functies waren goed.
• Hij was niet meer gaan werken, maar het was ook duidelijk dat hij al problemen met zijn werk had voor de circulatiestilstand.

• Redelijk goed hersteld (“goede CPC 2”).

Wel of geen epilepsie?
Sommige auteurs vereisen epileptische activiteit op het EEG voor de diagnose ‘status myoclonus’ ook wel ‘myoclonic status epilepticus’ Voor anderen is dat juist een reden om de diagnose te verwerpen.

Acute postanoxische myoclonus
Van de 391 patiënten in the PROPAC II studie, ontwikkelden 79 (20%) patiënten myoclonus.
Van deze 79 hadden 55 een primair cardiale circulatiestilstand en 8 hadden een primair hypoxisch arrest.
Uitkomst
Een goede uitkomst werd gezien in 11% (9 van 79) van de patiënten. Eén patiënt met slechte uitkomst bleef in leven.
SSEP in myoclonus
• SEP’s were available in 51/79 patients
• In 27 SEP’s (53%) N20 potentials were present.
• Giant potentials were seen in 3 patients.
EEG in myclonus
• 36 EEG’s: In 23 patients (64%) EEG criteria for a cortical origin were fulfilled.
• Epileptiform activity and/or status epilepticus were seen in 18/36 patients (50%).

• 9 EEG’s showed generalized periodic discharges
• Only 2 patients had a burst-suppression pattern on the EEG, both patients had a reported status myoclonus.

Patiënten die niet met hypothermie behandeld zijn en bij wie na 72 uur na reanimatie de pupilreacties OF corneareflexen afwezig zijn OF een beste motorische score ≤ 2, hebben een slechte uitkomst (overleden, vegetatieve toestand of ernstige invaliditeit na een half jaar).
Patiënten die met hypothermie behandeld zijn en bij wie na 72 uur na reanimatie de pupilreacties EN corneareflexen afwezig zijn, hebben vrijwel zeker een slechte uitkomst (overleden, vegetatieve toestand of ernstige invaliditeit na een half jaar). Deze factoren komen echter slechts bij 10% van de patiënten aanwezig en zijn dus slechts bij een klein deel van de patiënten behulpzaam bij het bepalen van de prognose.
De afwezigheid van contralaterale N20-toppen bij een beiderzijds onderzochte n. medianus SSEP, minstens 24 uur na de reanimatie en bij normothermie, voorspelt betrouwbaar een slechte prognose (overleden of vegetatief na een maand).
De aanwezigheid van een vlak EEG of een laag-voltage EEG (<10-20 microVolt) wijst zeer waarschijnlijk op een slechte prognose.
De betekenis van (continue) epileptische fenomenen op het EEG zonder klinische uitingen ten aanzien van de prognose – met of zonder behandeling - is nog onduidelijk.
De betrouwbaarheid van het vaststellen van een status myoclonicus wordt te gering geacht om deze variabele als een betrouwbare voorspeller van een slechte uitkomst te gebruiken

So, in all these papers including post-CA patients treated with TH, and studying the reliability of prognosticators of outcome in coma patients after cardiac arrest, the majority of patients that died of neurologic causes, treatment was withdrawn or limited, most probably based on confidence placed in prognosticators that were derived from studies not using a temperature management strategy.
