Only in Montréal does a simple stroll seem like time travel: you goback in time or leap to the future with every step. With memoriesof New France swirling amongst ultramodern construction, it’sno wonder that Montréal was the first North American city to benamed a UNESCO City of Design (2006).

Relive the romance of days gone by amidst Old Montréal’s cobblestone streets and 18th- and 19th-century stone buildings, taking time to gaze in awe at Notre-Dame Basilica.

Amble towards the present through Montréal’s new jewel of urbandesign—the Quartier International—and beyond to experienceour downtown’s eclectic mix of old and new: striking skyscrapers, heritage buildings, sleek cafés, designer boutiques and art galleries plus architectural innovations such as Place Ville-Marieand the Underground Pedestrian Network set the scene forMontréal’s cosmopolitan vibe.

The futuristic constructions such as Expo 67’s geodesicBiosphère, the Casino de Montréal, Habitat 67’s building-blockapartments and the Olympic Stadium are all testament toMontréal’s avant-garde vision.

Mount Royal, with its stately greystone buildings and Victorian-style mansions stand tall, while St. Joseph’s Oratory watchesover the city from its imposing perch.

Finally, Montréal’s neighbourhoods. Our signature wrought-ironstaircases, the “plexes” of many configurations (du, tri, four and five), plus the cultural touches of our diverse communities, reveal the true nature of our tremendous city.

Validation of RT-PCR Methods for MolecularDiagnostics and Epidemiology of Measles VirusJ. HIEBERT,* G. TIPPLESNational Microbiology Laboratory, Public Health Agencyof Canada, Winnipeg, MB

Objective: A measles vaccine has been licensed for usein Canada since the 1960s and there is a pan-Americaninitiative to eliminate measles from the Americas.Enhanced surveillance, which includes the case-by-caselaboratory confirmation of suspected measles cases, isimportant for monitoring the status of measles in Canada. The elimination of endemic measles was documented inCanada in 2004. Reverse transcription polymerase chainreaction (RT-PCR) for measles is important for both diagnostic and molecular epidemiological purposes. The objective of this study was to fully validate a sensitive, specific and rapid RT-PCR method for measles diagnostics.

Methods: Two real-time TaqMan probe based RT-PCRassays for the detection of measles virus RNA in clinicalspecimens were selected from the literature for evaluationin addition to our RT-PCR method used for genotypingpurposes. Dilution series of synthetic hemagglutinin (H)and nucleoprotein (N) gene RNA were tested a minimum of21 times with all three RT-PCR methods to determine theirdetection limits, reproducibility and repeatability. In addition,a panel consisting of clinical specimens and isolates (n = 65) that had been previously tested with another PCRmethod (27 negative and 38 positive) were tested.

Results: The detection limit, as defined as the minimum number of target copies that produce a positive result in95% of test runs, was found to be 100 copies of syntheticRNA per reaction for the two real-time RT-PCR assaysand the genotyping RT-PCR assay. The detection limitconcentration (100 copies for all three) was positiveevery time that it was tested (minimum of 7 separateruns of triplicates) resulting in reproducibility andrepeatability estimates of >95%. Finally, the panel of 65samples tested with these RT-PCR methods yielded theexpected results for all samples. The sensitivity and specificity are therefore estimated to be >95%.

Conclusion: The two real-time RT-PCR assays for measlesproved to be rapid, analytically sensitive, reproducibleand have high specificity and sensitivity. The use of twodifferent target genes (H and N) is desirable to avoidfalse negatives due to potential viral mutations.

We are looking for your interesting, difficult and head-scratchingcases. The session is interactive with the audience using touch– pads for input as they try to solve these tricky cases.

Please submit your most interesting clinical and laboratorycases for consideration by sending a brief outline of the case(including investigations, diagnosis and treatment) and a shortdiscussion on the instructive value of the case (about 200words). We like a variety of cases so they can be lengthy andcomplicated or quick and short “snappers” – you decide whatyou think might be of educational value to the audience. Thesecases will be the basis for discussion of clinical and laboratorydiagnosis, clinical management, infection control and other relevant aspects.

If your case is selected, we will work with you to prepare bothyour slide presentation for the meeting and an abstract of thecase (that does not give away the answer) that is suitable forpublication in the Canadian Journal of Infectious Diseases &Medical Microbiology (CJIDMM). Your slide presentation of thecase for the meeting will include the clinical presentation, investigations, and management, as well as the interactiveaudience questions.

The deadline for submission of clinical cases is January 10, 2011.Cases must be submitted by email to Dr. Michael Libman,[email protected]. Do not submit the cases throughthe regular abstract submission process.

If your case has been accepted for presentation, you will be notified on or around February 21, 2011. You can expect to beinvolved in presenting and discussing the case during theClinical Vignettes session. Those cases not selected for presentation may be accepted for poster presentation as a case report.

If you have any questions, please contact:Dr Michael Libman – [email protected].: (514) 934-1934 ext 42811

Steps away from the hotel, the cobblestone streets of Old Montréalpave the way for an exploration of the restaurants, galleries, boutiques and notable sites that epitomize Montréal’s history andjoie de vivre. Old Montréal is everything old, and everything new.
