Alhoewel de accreditering reeds 15 jaar bezig is, zijn er nog collega’s die het systeem blijkbaar niet snappen. Aanwezigheden bij cursussen en symposia dienen door de organisator doorgegeven aan het RIZIV. Dit gebeurt aan de hand van het RIZIV-nummer. Ook dient de aanwezigheid gevalideerd te worden door het aftekenen van de lijst bij het einde van de activiteit.Deze 2 cruciale onderdelen leiden tot frustraties bij ons, de inrichter. Collega’s die niet inschrijven, of laattijdig (er is steeds een limietdatum op de folder!), maken het wel vreselijk moeilijk. Het bijwerken van de lijst ad hoc geeft soms verkeerde nummers, het eigenhandig noteren van naam met daarnaast een handtekening moet dan dubbel gecontroleerd worden. Dan zijn er nog collega’s die zo haastig zijn om weg te kunnen, dat ze niet naast hun

naam aftekenen, maar vlug hem er ergens bijschrijven... Of nog erger, vergeten af te tekenen.Waarde collega’s, vanaf nu zullen we strikter zijn, met andere woorden zoals voorgeschreven. Indien er niet conform ingeschreven en afgetekend is, zal het nummer niet op de lijst bestemd voor het RIZIV voorkomen. Een niet correct aftekenen op de lijst, dit wil zeggen met naast de naam geen RIZIV-nummer, wordt niet doorgegeven. Foutieve nummers worden door het RIZIV niet aanvaard, wat logisch is.Als universitair geschoolden denk ik dat we na 15 jaar toch moeten weten dat dit zo dient te verlopen. De administratie is reeds enorm, maak het daarom ook vlot en correct uitvoerbaar in jullie eigen voordeel.

Update 2014

Encompassing a vast array of materials, today’s all-ceramic systems are suitable for a large range of indications. The use of lithium-disilicate- and zirconium-dioxide– ceramic based frameworks along with an identical veneering ceramic enables the dental care professional to cover almost all indications in fixed prosthodontics while achieving the same outstanding esthetic results.

The lecture describes clinical cases involving different indications to illustrate the use of different ceramic materials for single-tooth restorations, implants and complex cases. All stages of the restorative procedures from treatment planning with an analytic wax-up, transfer into a diagnostic template, selection of appropriate materials to adequate tooth preparation, laboratory fabrication and cementation will be included. Proven treatment concepts from A (Adhesive technique) to Z (zirconium-dioxide ceramic) will be provided in a step-by-step documentation.

All-ceramics from A to Z Update 2014: waarom een cursus?

Kroon en brugwerk, nu restauratieve tandheelkunde genoemd, evolueert met de

nieuwe materialen. Het kleven van restauraties is een nieuw gegeven dat zeer

tandsparend kan werken, dit in combinatie met de ceramische restauraties. Daar

zit nog steeds een evolutie in. Het gebruik van zirkonium is denk ik reeds zeer

verspreid, glasporselein minder. Onbekend is onbemind?

Zoals een professor ons laatst zei: het is eigenaardig dat de aanzet tot gebruik

van nieuwe materialen veelal door de labo’s wordt gepromoot, weinig of niet

door de tandarts. Het gebruik van nieuwe materialen en daaraan zonder meer de

nieuwe technieken bij het verwerken in de mond, leidt tot vele vragen en zonder

meer ook angsten voor mislukkingen.Deze uitgebreide cursus met toelichtingen bij

het gebruik van ceramische materialen zal de vele vragen beantwoorden en een

verantwoord gebruik van deze nieuwe materialen toelaten. Wanneer en hoe

verwerken, hoe de verantwoorde keuze maken welk ceramisch materiaal

geïndiceerd is bij welke restauratie. Dit kan niet in anderhalf uur gezien de

complexheid van de materie, daarom kozen we voor het cursusmodel: 60

accrediteringseenheden met bruikbare informatie.

Is the Evidence Supporting Dental Procedures Strong? A Survey of Cochrane Systematic Reviews in Oral HealthBackground: Every day a large number and variety of dental procedures are performed in clinical dental practice. There is, however, no information on the overall quality of evidence supporting these procedures. The objective of this study was to assess whether several common dental procedures are based on sound evidence.Methods: All Cochrane systematic reviews (CSR) published in dentistry were surveyed. The authors’ conclusions about the quality of evidence supporting a specific clinical treatment were used as the measure of outcome. The evidence was considered adequate if the authors did not clearly state the evidence was weak in the conclusions while also suggesting some evidence of the effectiveness of the therapy.Results: Of 120 CSRs assessed, in only 26 (22.0% of the reviews) was the quality of evidence regarded as adequate for supporting clinical decisions, although some methodological limitations were identified in the full text of these reviews. Moreover, the authors of most reviews reported weak or unavailable evidence.

Conclusions: On the basis of CSRs, the overall quality of evidence can be regarded as low or nonexistent for most of the dental procedures assessed. The information reported may guide future research.

The Connection Between Ruptured Cerebral Aneurysms and Odontogenic BacteriaBackground Patients with ruptured saccular intracranial aneurysms have excess long-term mortality due to cerebrovascular and cardiovascular diseases compared with general population. Chronic inflammation is detected in ruptured intracranial aneurysms, abdominal aortic aneurysms and coronary artery plaques. Bacterial infections have been suggested to have a role in the aetiology of atherosclerosis. Bacteria have been detected both in abdominal and coronary arteries but their presence in intracranial aneurysms has not yet been properly studied.

Objective The aim of this preliminary study was to assess the presence of oral and pharyngeal bacterial genome in ruptured intracranial aneurysms and to ascertain if dental infection is a previously unknown risk factor for subarachnoid haemorrhage.

Methods A total of 36 ruptured aneurysm specimens were obtained perioperatively in aneurysm clipping operations (n=29) and by autopsy (n=7). Aneurysmal sac tissue was analysed by real time quantitative PCR with specific primers and probes to detect bacterial DNA from several oral species. Immunohistochemical staining for bacterial receptors (CD14 and toll-like receptor-2 (TLR-2)) was performed from four autopsy cases.

Results Bacterial DNA was detected in 21/36 (58%) of specimens. A third of the positive samples contained DNA from both endodontic and periodontal bacteria. DNA from endodontic bacteria were detected in 20/36 (56%) and from periodontal bacteria in 17/36 (47%) of samples. Bacterial DNA of the Streptococcus mitis group was found to be most common. Aggregatibacter actinomycetemcomitans, Fusobacterium nucleatum and Treponema denticola were the three most common periodontal pathogens. The highly intensive staining of CD14 and TLR-2 in ruptured aneurysms was observed.

Conclusions This is the first report showing evidence that dental infection could be a part of pathophysiology in intracranial aneurysm disease.

Study Shows Invisalign More Profitable Than Braces

Invisalign clear plastic orthodontic aligners cost more in materials than conventional edgewise braces, but they require fewer patient visits and a shorter duration of treatment, researchers reported here at the American Association for Dental Research 2012 Annual Meeting.

Both approaches to moving teeth are useful, first author Michael Ross, a dental student at the Baylor College of Dentistry in Dallas, Texas, told Medscape Medical News. "It's weighing the time cost against the material costs," he said.

In the Invisalign system, practitioners fit their patients with a series of plastic aligners fabricated in a laboratory that incrementally adjust the teeth in the desired direction. The system has allowed some general dentists to do what only orthodontists could do before. But few studies have measured which approach is more efficient in time or money.

To explore this question, Rossi and his colleagues evaluated the records of 150 patients with mild to moderate class I malocclusion. The 2 groups were matched for the amount of initial malocclusion and the number of rotated teeth.

The researchers measured the time taken by appointments for both types of treatment with a stopwatch.

They found that conventional braces required a median of 13.6 minutes for a routine visit, whereas Invisalign required 9.9 minutes. Emergency, initial, and final appointments were all longer than regular visits.

Conventional braces required about 2.6 more visits than Invisalign, treatment for 2.4 months longer, 1.1 more emergency visits, 9.7 minutes more in chair time, 1.2 minutes more emergency doctor time, and 86.2 minutes more in total chair time (P < .01 for all).

However, Invisalign cost $500 to $1441 more in materials and required 5.9 minutes more doctor time than conventional braces (P < .01).

Measuring profitability as fees minus the cost of materials, Invisalign was more profitable than conventional braces, the researchers found, especially for Invisalign providers who are charged $899 in lab costs, a discount that the appliance maker, Align, offers to doctors who do more cases.

For these doctors, Invisalign provided about $1000 of profit per hour of chair time with the first $2750 in fees to the patient, rising to $3250 with $6000 in fees. Doctors who are charged $1549 in lab costs by Align had a profit of about $500 with the first $2750 in fees, rising to $3000 with $6000 in fees.

Braces were less profitable overall, starting at $750 per hour of chair time with the first $2750 in fees, but reaching only $2000 with $6000 in fees. (nvdr: dit zijn geen Vlaamse prijzen...)

Asked to comment, Allen Firestone, DDS, MS, professor of orthodontics at Ohio State University in Columbus, told Medscape Medical News that doctor time could vary a lot from doctor to doctor; all the records in this study were obtained from one orthodontist's office.

Dr. Firestone, who was not involved in the study, uses both treatments in his own practice. "Invisalign is a very effective way of treating a broad range of orthodontic problems," he said, "but you have to choose your patients."

He explained that Invisalign is more esthetic because the clear aligners are hard to see, are less irritating to the tissue, and allow better hygiene because they can be removed.However, it relies more on patient compliance, and some tooth movements are more difficult, he said. "As an orthodontist, I can use braces on some teeth and Invisalign on others," he said. "You want a practitioner who is using Invisalign because it's good for the patient, not because it's good for the practitioner."

BTW en de VZW

Zoals jullie konden zien bij de aankondigingen van de ICT en het symposium is een nieuwe regeling van kracht.Er is ons gewezen op de BTW-plicht in Europese context. Ook een vereniging zonder winstgevend doel moet hieraan voldoen. Maar België zou België niet zijn, moesten er geen uitzonderingen bestaan. Deze wordt gemaakt als een vereniging zonder winstgevend doel enkel voor de leden werkt en de gelden die deze ter beschikking stellen enkel voor de werking worden gebruikt.De keuze is in deze snel gemaakt. Ik denk dat niemand staat te springen om 21% extra te betalen?
