Neurological gait disorders in elderly people.
Lancet Neurol 2007;6:63-74
Frequentie
• Gangstoornissen zijn frequent bij ouderen
en hun prevalentie stijgt met de leeftijd
– Op 60 jaar, nog 85% normaal gangpatroon
– Op 85 jaar, slechts 18%
• Het valrisico neemt toe met de leeftijd
– >65 jaar:
• 1/3 valt 1x/jaar
• helft hiervan >1x/jaar
Belang
Gevolgen:
Vallen
Immobiliteit en verhoogde afhankelijkheid
Verminderde levenskwaliteit
Normaal
verouderingsverschijnsel?
Belang
• Marker voor cardiovasculair risico en
dementie
– Vroegtijdig teken van cerebrovasculair of
neurodegeneratief lijden
Wandelen: complex?
• Voortbeweging

2. Neen
“Stops walking when talking”
• Lundin-Olsson et al. Lancet 1997;349:61
“Stops walking when talking” as a predictor
of falls in elderly people.
• Toonden aan dat ouderen die stoppen om
te praten een veel hoger valrisico hebben
• Bijgevolg geen automatisme
– Automatisme: bijkomende taak geen invloed
The role of executive function and attention in gait.
Mov Disorders 2008;23:329-342
Executieve functies
• Gangsnelheid is afhankelijk van
– Executieve Functies
– Geheugen
– Niet van verbaal IQ
• De associatie is belangrijker zo:
– Moeilijkere motorische taak
– Aangetast gangpatroon door ziekte (bv. PD)
The role of executive function and attention in gait.
Mov Disorders 2008;23:329-342
Executieve functies
• EF wordt bepaald door een netwerk van
anterieure en posterieure regio’s
• EF > frontale cortex
• Frontale cortex > andere regio’s
• Frontale cortex gevoelig aan
leeftijdseffecten
