(1) When the origin declaration is completed for multiple shipments of identical

originating products within the meaning of Article 19.5 indicate the period of time for

which the origin declaration will apply. The period of time must not exceed 12 months.

All importations of the product must occur within the period indicated. Where a period

of time is not applicable, the field can be left blank.

(2) For EU exporters: When the origin declaration is completed by an approved or

registered exporter the exporter’s customs authorisation or registration number must

be included. A customs authorisation number is required only if the exporter is an

approved exporter. When the origin declaration is not completed by an approved or

registered exporter, the words in brackets must be omitted or the space left blank.

For Canadian exporters: The exporter’s Business Number assigned by the Government

of Canada must be included. Where the exporter has not been assigned a business

number, the field may be left blank.

(3) “Canada EU” means products qualifying as originating under the rules of origin of

the Canada-European Union “Comprehensive Economic and Trade Agreement. When

the origin declaration relates, in whole or in part, to products originating in Ceuta and

(5) Article 19.3 provides an exception to the requirement of the exporter’s signature.

daaropvolgende houder van de goederen die een dergelijke verklaring opstelt zijn naam

en volledig adres gevolgd door de vermelding (Engelse versie) ‘acting on the basis of

the statement on origin made out by [naam en volledig adres van de exporteur in het

LGO], registered under the following number [registratienummer van de exporteur in

het LGO]’.

(2) Aanduiding van het land van oorsprong van de producten. Indien het attest van

oorsprong geheel of gedeeltelijk betrekking heeft op producten van oorsprong uit Ceuta

en Melilla in de zin van artikel 46 van deze bijlage, moet de exporteur deze producten

door middel van de letters ‘CM’ duidelijk aangeven op het document waarop de

verklaring wordt opgesteld.

(3) Geheel en al verkregen producten: vermeld de letter ‘P’; producten die een

voldoende be- of verwerking hebben ondergaan: vermeld de letter ‘W’ gevolgd door de

viercijfercode van het geharmoniseerd systeem waaronder het uitgevoerde product is

ingedeeld (voorbeeld ‘W’ 9618); indien van toepassing wordt bovengenoemde

vermelding vervangen door een van de volgende vermeldingen:

a) bij cumulatie op grond van artikel 2, lid 2, van deze bijlage of bilaterale cumulatie op

grond van artikel 7 van deze bijlage: ‘EU cumulation’ of ‘cumul UE’; ‘OCT cumulation’ of

‘cumul PTOM’;

b) bij cumulatie met een EPO-land op grond van artikel 8 van deze bijlage: ‘cumulation

with EPA country [naam van het land]’ of ‘cumul avec le pays APE [naam van het land]’;

c) bij cumulatie met een SAP-land op grond van artikel 9 van deze bijlage: ‘cumulation

with GSP country [naam van het land]’ of ‘cumul avec le pays SPG [naam van het land]’;

d) bij cumulatie met een land waarmee de Unie een vrijhandelsovereenkomst heeft op

grond van artikel 10 van deze bijlage: ‘extended cumulation with country [naam van het

land]’ of ‘cumul étendu avec le pays [naam van het land]’.
