Hoe werkt braken?
Mechanismen van chemotherapy-induced nausea and vomiting (CINV):

Anti-emetica : medicatie
● 3 soorten:
●Gastroprokinetica: vb. Alizapride, domperidon, metoclopramide
●5HT3-antagonisten: vb.ondansetron
●NK1-antagonisten: vb. aprepitant
●BIJWERKINGEN
Hoe werkt acupunctuur ?
hersenen.
Aangetoond met cFos kleuring (World Journal of
Gastro-Enterology. Beijing University. 2007;13: 4873-

Acupunctuur
●
Mechanisme oncologie volgens TCM :

Onderzoeksvraag

Resultaten
Resultaat dichotome data (werkzaamheid per acustimulatie techniek)
1.41 (1.06, 1.87)
2.09 (1.42, 3.06)
1.65 (1.45, 1.89)
1.73 (1.43, 2.08)Overall

Resultaat continue data:
Per uitkomst maat voor nausea, braken, gecombineerd effect
Mean: niet significant
Voornamelijk studies met 1 punt
Ongewenst effecten: weinig

Literatuuroverzicht ivm chemotherapie
● Volgens Cochrane kan CINV ingedeeld worden in 3
klinische fasen:
1. ANV (anticipatory nausea and vomiting):
2. acute NV: within 24 hours
3. delayed NV: after 24 hours

An update from the PubMed Database
Radiotherapy and chemotherapy are often associated
with severe nausea and vomiting.
First article on the subject: 1970.
1997: consensus the National Institute of Health (NIH-
USA) to evaluate its safety and efficacy.
The panel stated : “there is clear evidence the
acupuncture treatment is effective in postoperative and
chemotherapy nausea and vomiting”.
● Ezzo et al. 2006 (Cochrane)
Review acu-point stimulation for chemotherapy induced
nausea or vomiting
● All sorts of acupuncture technics were analysed : 11
studies have been maintained with N=1247
● Less nausea, especially the acute nausea
● but the severity of the nausea = the same.
● Pc6, ST36
●
Results Ezzo et al. Cochrane Review
Acupuncture can help
● In the acute nausea and vomiting with any kind of
acupuncture
● In the proportion of NV
● But not in the severity
● And not in delayed NV

Current evidence on Auricular Therapy for chemo-therapy induced
nausea and vomiting in cancer patients (review)
● Auriculo in combination with anti-emetic drugs is
more effective than drugs alone with 87,50% in the
intervention group and 45% in the control group.
● auricular points:
● stomach, liver, spleen, sympaticus, shenmen

(Rochester, NY,USA)
Anticipatory nausea and vomiting due to chemotherapy( related
to psychological processes)
● More succes with acustimulation when used alone or
in combination with anti-emetic drugs or counseling;
● No evidence of utility in anticipatory nausea and
vomiting.

Efficacy of acupuncture in prevention of delayed chemotherapy induced
nausea and vomiting in gynecologic cancer patients
● Comparing acustimulation and ondansetron we have
better results for acu (52,8% versus 35,7%, p= 0,02) en
less side-effects such as constipation and sleepyness.
● Pc6
● Demet Tas, 2014
Acupuncture as a complementary treatment for cancer patients
receiving chemotherapy
● The study showed acupuncture had positive effects in
cancer treatment by patients with nausea, vomiting,
pain, poor sleep and anxiety as side effects; these could
be decreased by acupuncture
● MC Culloch Renee et al, 2013Symptom management during chemotherapy in children
●She discussed pain,mucositis,poor eating,fatigue,sleepyness,negative
mood and nausea and vomiting
● Phillips et al (2011) reviewed 28 trials of 23 drugs : 5-

2. evaluate Risk of Bias (ROB)
3. synthesis of conclusion of the included studies
by symptom (NV)
4. proposition for studies to come
There was significantly good results for electro-acupuncture
even in comparison with the medication group and so
acupuncture is an appropriate adjunctive treatment for
chemotherapy induced nausea/vomiting.

Conclusion
● By study of the most recent literature on the subject
we can conclude that there is
● Sufficient evidence that acupuncture, especially Pc 6
and ST36, can be a very good alternative to
medication, without side effects, for treating the CINV.
●A clinical study with a rigourous methodology and a
great number of participants would be interesting for
the future
