Achievements in Clinical Excellence:Using data-powered insight to improve behavioral health care

On improving behavioral health care

RESOURCE CENTER FOR HEALTH & WELL-BEING

Quality firstHospitals and behavioral health facilities must pass a rigorous credentialing process to become part of the Optum network. Once a facility enters the ACE program, it is assigned to one of five tiers, “Platinum” being the top.

While all facilities in the Optum network must meet rigorous standards, Platinum facilities are indicated with two stars and Gold facilities with one star next to their names in online provider directories.

The Optum™ Achievements in Clinical Excellence (ACE) program uses advanced data analytics to rate the performance of behavioral health treatment facilities for quality of care and efficiency in treatment. Consumers can see which inpa-tient facilities are rated most highly when they search online directories. Optum takes ratings even further, however, by working with inpatient facilities on ways to improve care for individuals with mental health or substance use disorders.

What ACE does:• Uses a data-driven approach to improve facility performance

• Identifies facilities providing the most effective and efficient care

• Monitors metrics for continuous improvement

• Recognizes and rewards top-performing facilities

• Increases referrals to the top facilities

What’s on the ACE score card?Seven key performance indicators: five that measure “effectiveness” (for quality of care), and two that measure cost “efficiency” — all based on widely accepted, industry standards for:

Ratings take into account the mix of patients for each facility, and use benchmarks that compare them to facilities in their areas.

• Readmission

• Follow-up with mental health practitioners after hospitalization

• Peer review rates

• Spend per inpatient episode

• Residual length of stay

Meets/exceeds efficiency criteria, but not peer review (even if facility meets other effectiveness metrics)

Criteria not met: Does not meet effectiveness metrics or efficiency criteria

ACE at a glance

• Provides unbiased, risk-adjusted data to behavioral health facilities that are quantified against regionally adjusted benchmarks

• Pinpoints and reduces unwarranted variations in practice patterns that drive increased costs and suboptimal clinical outcomes

• Assigns tier designations to facilities and rewards those with superior performance

The result: Better behavioral health care

Platinum status has its rewards The goal of ACE is to help all facilities reach Platinum status and receive its benefits:

• Two-star designation in the Optum online provider directory*

• Sharing best practices and how others achieved Platinum status

• Encouraging “bridge” appointments with clinicians after discharge but before patients leave an acute inpatient unit

• Help matching patients on discharge to individuals who are in recovery and trained as peer supports

• An appointment reminder program — email, voice or text messaging — to remind discharged patients about their follow-up appointments

More than ratings: Support for improving care Optum offers assistance and consultation to behavioral health facilities ranked below Platinum. That support may include:

OPTUM Product Brochure

Relapse doesn’t mean treatment has failed. Addiction is a chronic illness, and relapse rates for alcohol and drug use are similar to other chronic conditions such as diabetes, high blood pressure and asthma. 2 Like these conditions, addiction has both physiological and behavioral components. Relapse doesn’t mean failure. It means taking another look at what’s working and what isn’t in terms of your treatment plan.3

Identifying cluesPeople who relapse may be able to look back at their experiences and find clues as to how it hap-pened. Relapse clues, or warning signs, may relate to changes in your behavior, attitudes, feelings, thoughts or a combination of these. This does not necessarily

mean that changes you experience are an indication that you may be in a relapse. It simply means that you should be on the alert when changes occur, and examine whether or not these do, in fact, indicate you may be headed for a relapse. The following are examples of “relapse clues” to watch for. 5

• Behavior changes– Arguing with people for no apparent reason

– “Forgetting” to take Antabuse

– Decreasing or stopping AA/NA (Alcoholics Anonymous/ Narcotics Anonymous) meetings

– Stopping in a bar to socialize and drink soda

– Increased stress symptoms such as smoking more or eating more

• Attitude changes– Not caring about staying sober

– Not caring what happens

– Becoming negative about life and how things are going

• Changes in thoughts– Thinking alcohol or drugs were “deserved” due to being sober six months

– Thinking it wouldn’t be harmful to substitute one drug for another (for example, giving up cocaine, but continuing to smoke marijuana; abstaining from alcohol but continuing to use uppers)

– Thinking the alcohol or drug problem was “cured” because no substances were used for a period of weeks or months

Continued on Page 2 »

Helpful strategiesThe following are ways to avoid relapse4:

• Identify the clues that you may be headed toward relapse. Increase or resume 12-step meeting attendance, and share what you are feeling.

See page 2 for resources for finding Alcoholics Anonymous or Narcotics Anonymous meetings in your area.

• Talk about your warning signs with your sponsor and create an action plan to maintain your sobriety and recovery.

• Become more involved in your home group.

• Get the phone numbers of people at the meetings and call them for support.

• Stay away from social situations where alcohol may be available.

• Socialize outside of meetings with people who are also in recovery.

Recovery from addiction: Avoiding relapse“Relapse” refers to using alcohol or drugs after a period of not using. It can happen even if a person has been sober for a long time.1 Learning about relapse should be part of any recover plan, so you can help prevent it from occurring by watching for warning signs.

• Changes in feelings or moods– Increased moodiness or depression

– Strong feelings of anger at oneself or another

– Increased feelings of boredom

– Sudden feelings of euphoria

These are just a few examples that may or may not relate to you. The important point to remember is that changes in your behaviors, attitudes, feelings, thoughts or a combination of these could indicate that your relapse process is set in motion.
