    This application document checklist details the information and supporting documents(personal, health and character) required to assist with the lodgement of a completeapplication with the department. You may also need to provide additional information anddocumentation after you have made your application if the department requires it.

    Complete applicationsYou should ensure that you lodge a completed visa application. While the departmentmay ask you to provide more information in order to determine whether you meet the

    criteria for grant of the visa, a decision on the visa application may be made solely on theinformation provided at the time of application.Note: The Visa Application Charge will not be refunded if a decision is made to refuse togrant the visa because the applicant did not satisfy the criteria for grant of the visa.

    Forms, fees and charges Form 47OFApplication for migration to Australia by other families

    A completed form 47OF. The form must be signed in all the relevant places andeach person included in the form, who is aged 18 years or over, has signed theAustralian values statement.

    Form 40Sponsorship for migration to Australia

    A completed form 40 from your sponsor.

    Form 47ADetails of child or other dependant family member aged 18 years or over

    A completed form 47A.

    Personal documents Documents to prove your identity a certified copy of your birth registration showingboth parents names. If you do not have a birth certificate and are unable to get one,you must provide a certified copy of the identification pages of at least one of thefollowing documents:

    passport family book showing both parents names identification document issued by the government documents issued by a court that verifies your identity.

    If none of these documents can be provided, other acceptable evidence that you arewho you claim to be must be submitted.

    A completed form 47A providing details of children and all other dependant familymembers aged 18 years or over, and evidence of their dependency on you and/oryour partner.See:Form 47ADetails of child or other dependant family member aged 18 years orover.

    Evidence of your relationship with the sponsor, (certified copy of birth certificate,marriage certificates, death certificates, adoption certificates, or family statuscertificates or family books if these documents are officially issued and

    maintained).

    Evidence that the sponsor is an Australian citizen, Australian permanent resident oreligible New Zealand citizen, (certified copy of birth certificate, Australian passport orforeign passport containing evidence of permanent residence), for the New Zealandcitizens evidence of length of residence in Australia and of continuing links withAustralia.

    Evidence of dependency for all dependants aged 18 years or over. Documents thatshow financial dependency may include bank records or other financial statements.

    If anyone included in the application is, or has been married, certified copies of themarriage certificate(s). If you are living in a de facto relationship, independent

    evidence that you have been in the relationship for at least 12 months (for examplejoint bank account statements, billing accounts in joint names).

    If you or anyone included in the application has been permanently separated,divorced or widowed, a certified copy of the statutory declaration/separationcertificate, divorce decree absolute or the death certificate of the deceased partner(as appropriate).

    If anyone in the application has changed his or her name (for example by marriageor deed poll), a certified copy of evidence of the name change.

    If any dependant child included in the application is adopted, certified copies of theadoption papers.

    Four recent passport size photographs (45mm x 35mm) of you and any other personincluded in the application.

    These should be of the head and shoulders only against a plain background.

    Print the name of the person on the back of each photograph.

    If you have served in the armed forces of any country, certified copies of militaryservice record or discharge papers
